This phase focuses on the critical steps of applying for your Texas Assisted Living Facility (ALF) license with HHSC, preparing your physical facility to meet regulatory standards, and establishing initial operational frameworks.
Key Objective 1: HHSC ALF Licensing Application
Navigate the comprehensive application process outlined in TAC §553, Subchapter B (Licensing Application and Issuance).
- Gather all required documentation: business entity information, proof of financial solvency, management qualifications, floor plans, etc.
- Complete and submit Form 3720 (Application for State License to Operate an Assisted Living Facility) and associated addenda.
- Pay applicable licensing fees.
- Understand the review process, potential for deficiencies, and timelines.
- Address any HHSC requests for additional information promptly and thoroughly.
Key Objective 2: Physical Plant & Safety Standards
Ensure your chosen facility meets or is modified to meet the stringent physical plant and safety standards found in TAC §553, Subchapter D, Division 7 (Physical Plant and Construction Requirements).
- Requirements vary based on facility Type (A or B) and capacity.
- Address resident rooms, common areas, bathrooms, kitchen, fire safety systems (alarms, sprinklers, extinguishers), and emergency exits.
- Ensure compliance with accessibility standards (ADA).
- Obtain necessary local building permits for any modifications.
Key Objective 6: Alzheimer's/Dementia Unit Certification (If Applicable)
If planning to operate a certified Alzheimer's/Dementia unit, prepare for additional requirements under TAC §553, Subchapter C, Division 5.
- Specific physical plant design features.
- Specialized staff training and qualifications.
- Activity programming tailored to residents with dementia.
- Application for certification is separate but often concurrent with the main ALF license.
